What happens during the disease - arthritis, unspecified

Подострая и хроническая форма артрита приводит к гипертрофии ворсинок синовиальных оболочек, патологической пролиферации (разрастанию) поверхностного слоя синовиальных клеток, плазмоцитарной и лимфоидной инфильтрации тканей с исходом в фиброз. При длительном артрите отмечается развитие грануляций на суставных поверхностях хряща, их постепенное распространение на хрящевую ткань, деструкция и эрозирование костно-хрящевого лоскута. По мере замещения грануляционной ткани фиброзной происходит процесс оссификации, т. е. формирование фиброзных или костных анкилозов. При заинтересованности суставной капсулы, сухожилий и околосуставных мышц развиваются деформации суставов, подвывихи, контрактуры.

 

В зависимости от нозологической формы при артрите поражаются различные группы суставов. Для ревматоидного артрита характерна симметричная заинтересованность суставов стоп и кистей - пястно-фаланговых, межфаланговых, лучезапястных, плюснефаланговых, предплюсневых, голеностопных. Псориатический артрит характеризуется поражением дистальных суставов пальцевых фаланг стоп и кистей; анкилозирующий спондилоартрит (болезнь Бехтерева) – суставов крестцово-подвздошного сочленения и позвоночника.
